ISO/IEC 17021-1 requires a competent person, not involved in the audit, to review the audit and its report before any certification decision. ISO 9001 is the highest report volume of any standard, and keeping consistency across auditors and reviewers by hand is hard. The most common nonconformities, requirements review, supplier evaluation, calibration, competence records, management review and root-cause analysis, recur, and reports fail when a nonconformity is written without the requirement, or without distinguishing correction from corrective action.
